New Patient Registration
Durnford Medical Centre is open to eligible NHS patients who wish to register with us. All information given to the practice is treated as confidential and would only be disclosed within the legal requirements of the data protection act. The practice policy is to accept any eligible person and not to discriminate against anyone, regardless of any pre-existing health conditions, required treatment, age, gender, race, ethnicity, religion, disability, sexual orientation or gender identity. Durnford Medical Centre received the "Pride in Practice" Gold award in June '13 for inclusion of LGBT patients, the certificate is displayed on the reception desk.
If you live within our practice area (roughly a three mile radius from the practice) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded from the above "Registering Online" tab.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Changes to Personal Details
If you change your name, address, or telephone number please tell us. Sometimes we may need to contact you and if we do not have your current details this will cause delay or maybe even make it impossible for us to contact you.
You may inform us by post or by calling in to fill in a form at reception. Make sure that you tell us which members of your household have moved with you - we cannot assume that all have moved with you since sometimes families split up or children move away to college etc.
Please tell us your telephone number even if you are ex-directory. We will not disclose it to anyone, but it is sometimes important that we are able to contact you quickly.
Note that if your new address is outside our practice area we may not be able to keep you as a patient at this practice. In that case we shall contact you to let you know that you need to find a practice in your new locality.
